Position Description

Title: Licensed Practical Nurse

Location: Northeast Region (Onsite)

Employment Type: Full Time, Days & Evenings: 8:00am - 4:00pm & 4:00pm - 12:00am

Compensation

Pay Range: $40.00 Per Hr

Description

Join a team of healthcare professionals in a supervisory role.
Engage in community nursing within adult care facilities.
Oversee and direct Home Health Aides.
Manage medication and nursing procedures for residents.
Promote a positive and safe living environment for residents.

Key Responsibilities

Oversee neighborhoods and manage Home Health Aides.
Deliver nursing care per agency policies.
Participate in incident reporting and follow-up actions.
Assist with resident admissions and conduct meetings.
Prepare and evaluate nursing care plans.
Observe and report changes in residents' conditions.
Foster a positive working environment and address employee concerns.

Qualifications

Graduate of an accredited practical nursing program.
Valid License as a Licensed Practical Nurse (LPN).
1-2 years of experience in community nursing, acute care, or adult care facilities.
Supervisory experience and medication management preferred.
