Job Description:
The Registered Nurse—Wound Care will conduct health assessments focusing on wound, ostomy, and continence care. The nurse will also perform focused physical assessments and develop treatment plans tailored to each patient's diagnosis and individual needs. Responsibilities include recommending medications and treatments, assisting in the selection of medical supplies and equipment, and documenting patient responses to care.
Key duties include:
- Wound Care Assessment: Perform comprehensive wound care, including ostomy, continence, fistula, and complex wound treatments, documenting patient responses and outcomes.
- Patient & Family Education: Educate patients and families about wound care management, complications, and prevention strategies.
- Staff Education & Training: Develop and evaluate training programs, including orientation and continuing education sessions.
- Collaborative Care: Work closely with the healthcare team and make referrals to other disciplines when necessary.
- Preventative Measures: Routinely assess patients at risk for developing pressure injuries and initiate preventative measures.
- Quality Improvement: Participate in quality improvement initiatives, including Incidence and Prevalence Studies, Nursing Quality initiatives, and hospital-wide activities.
- Cultural Competence: Provide culturally competent care and education in alignment with the patient’s and family's cultural, ethnic, and religious beliefs.
- Community Resources: Identify and refer patients to community resources to support their wound care and other health needs.
- Equipment Management: Oversee asset management related to rented equipment, including specialty beds and Negative Pressure Devices.
Travel Requirement:
- Approximately 70% of the role will be in-office, with the remainder requiring travel to patient homes across Manhattan (MA), Brooklyn (BKLYN), Bronx (BX), and Queens (QU) for wound care consults.
- On average, the nurse will handle 15-20 consults per day.
Certifications Required:
- Current New York State RN License
- Primary Source Verification
- AHA BLS (Basic Life Support) Certification
- One of the following wound care certifications:
- Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB
- Certified Wound Ostomy Nurse (CWON) – WOCNCB
- Certified Wound Care Nurse (CWCN) – WOCNCB
- Certified Wound Specialist (CWS) – ABWM
- Wound Care Certified (WCC) – NAWCO
Experience Required:
- Minimum of 2 years of med/surg experience
- Experience with wound and colostomy care
